Senior Outreach Services has three case managers on staff to assist Seniors in our Community and those of the surrounding areas. A case manager is a resource for n information related to a variety of topics.
We assist many seniors with obtaining ad maintaining financial assistance such as Food Share, Energy Assistance, Medicare including Medicare Prescription Coverage and Medicaid. If the person also has complex or ongoing medical issues we may discuss services such as Care WI or Hospice Services.
For Caregivers we provide support, education and respite funding including linking them with private pay caregivers or publically funded long term care.
Our goal is to keep people in their home if possible so we work with partner agencies such as Home Health. We may also provide loan closet equipment like wheelchairs and bath benches for short term use. We encourage good nutrition to maintain your health by providing a meal site for seniors, Meals on Wheels for homebound seniors’, bus rides to the grocery store and connection with the McFarland Food Pantry.
Our case managers partner with law enforcement and medical emergency services to assure the safety and wellbeing of seniors in their home. We hope to put preventative services in place such as when a fall or injury has occurred or there are emotional or behavioral concerns such as depression. Through resources and other programs offered here or elsewhere we try to connect people with the social supports they need and want.
The case managers also have information related to a variety of other resources such as housing, estate planning, emergency preparedness, home safety, abuse and neglect.
