Please call our offices to discuss your concern. Priority will be given to those affected by this crisis; other functions of Outreach and Case management will be addressed as time allows.

Case Management

Our case management is a Dane County Contracted service provided to seniors over the age of 60 in McFarland, Town of Dunn, Town of Pleasant Springs, Village of Cambridge, Rockdale, and Town of Christiana.

McFarland Senior Outreach has two case managers who develop on ongoing relationship with a client to access services over an extended period of time. Services may include linkage to financial programs such as food share, energy services, or accessing long-term support through Dane County. We frequently work with caregivers and families on developing long-range plans when the senior is faced with a life altering health concern such as dementia or cancer.

Services are available to anyone over the age of 60, but often times outreach services lead to a more defined relationship that becomes a case management relationship over time.
