SAFE at Home Helps Reduce Falls

SSM Health Safe at Home provides comprehensive care for patients who are high risk for falling. Per the CDC (Centers for Disease Control and Prevention), one out of three older people fall each year, but less than half tell their doctor. Chances of falling doubles once you have already taken a fall. Our program, SAFE at Home provides home safety assessments and medication reviews for Dane County seniors ages 60+. Our goal is to reduce falls and help participants live as safely and independently in their own homes for as long as possible.

Risk factors for falling include:

  • Muscle weakness
  • Poor balance
  • Drop in blood pressure
  • Foot problems / improper footwear
  • Vision issues
  • High number of medications
  • High risk medications
  • Sensory or sensation issues
  • Memory or cognitive issues
  • Recent illness or surgery
  • Previous fall
  • Environment - obstacles, lack of safety equipment, poor lighting

SAFE at Home provides:

  • Basic in-home safety assessment
  • Medication review by a pharmacist
  • Safety aids including a pillbox and nightlight
  • Home safety recommendations
  • Information about community resources
  • Six months of follow-up by a medical social worker
