What is case management and outreach?

Case management is helping older adults navigate through a maze of information and identify resources to promote wellness. A case manager has an ongoing relationship with the client to monitor their usage of services over an extended period of time and advocate for additional services as needed or indicated.

Outreach services are defined as providing short term or one time information and assistance to seniors about programming, loan closet, or outside resources. This includes the programs offered regarding:

  • Caregiver resources
  • Dementia-specific concerns
  • Exercise classes
  • Financial resources
  • Foot clinics
  • Legal and financial planning
  • Seasonal tax clinics
  • Social and/or support groups
