1. Who does McFarland Senior Outreach serve?

For case management services, we serve seniors over the age of 60 residing in:

• Cambridge, Village of (Dane County)
• Christiana, Town of
• Dunn, Town of
• McFarland, Village of
• Pleasant Springs, Town of
• Rockdale, Village of

Our meal sites our open to any senior over the age of 60. Home-delivered meals are provided for qualified seniors in the Village of McFarland, Town of Dunn, and Town of Pleasant Springs.

2. What is case management and outreach?

Case management is helping older adults navigate through a maze of information and identify resources to promote wellness. A case manager has an ongoing relationship with the client to monitor their usage of services over an extended period of time and advocate for additional services as needed or indicated.

Outreach services are defined as providing short term or one time information and assistance to seniors about programming, loan closet, or outside resources. This includes the programs offered regarding:

• Caregiver resources
• Dementia-specific concerns
• Exercise classes
• Financial resources
• Foot clinics
• Legal and financial planning
• Seasonal tax clinics
• Social and/or support groups

3. Does Senior Outreach collaborate with the Village departments on services to seniors?

• We work with emergency medical services (EMS) staff on teaching evidenced based classes, such as Living Well with Chronic Conditions, Powerful Tools for Caregivers, and Healthy Living with Diabetes. We distribute Files of Life to seniors for EMS to access pertinent medical information in an emergency. We also offer Knox Boxes for access into the home in case of an emergency.
• Our police department offers the "Are You Okay" program, which serves vulnerable seniors. Our police officer and our case manager partner on visits and interventions for seniors who experience mental health, dementia, and legal risks through ongoing supports.
• We also plan programming with the E.D. Locke Public Library staff.
• The other technology that is used is a Care Trak system for adults at risk of wandering due to cognitive or dementia related behaviors.

4. What nutrition services does Senior Outreach offer?

5. Does Senior Outreach provide transportation for shopping?

We provide a shopping bus five times a month. Two trips are dedicated to going to the local Pick N’ Save, one trip to the Walmart in Monona, and one trip the destination varies. You may also request an individual shopping trip on a select day once a month.

6. Does Senior Outreach provide rides for medical appointments?

We coordinate rides for medical appointments under the Retired Senior Volunteer Program (RSVP). Screened volunteers handle rides. Reservations are required 1 week in advance.

Since this is a volunteer-based program, we make every effort to find a driver but cannot guarantee a ride. Rides must be during regular business hours of 8 am to 4:30 pm, and you must register as an RSVP passenger. There are some limitations on rides for ongoing appointments such as infusion and dialysis. Cost is on a donation basis.

7. Does Senior Outreach have a medical loan closet?

Yes, we have a medical loan closet for short-term use. We have items such as:

• Bath seats and transfer benches
• Bed rails
• Commodes
• Standard and transport wheelchairs
• Toilet rails
• Walkers
• Other miscellaneous items

8. What is the Foot Clinic?

The foot clinic provides toenail clipping for seniors, both diabetic and non-diabetic, who require assistance in trimming their own nails. For more information, visit our Foot Care Clinic page.

9. Does Senior Outreach Services provide volunteer opportunities?

Our highest need is always Home Delivered meal drivers and Retired Senior Volunteer Program (RSVP) medical drivers. If you are interested in these or other opportunities please contact our Volunteer Coordinator/Case manager, Katie Gletty-Syoen, at 608-838-7117.

10. How do we find out about events and programs?

We publish a monthly newsletter, available free online. If you wish to subscribe to a hard copy or to have one sent to you via email, please call 608-838-7117. Copies are available in the Village of McFarland municipal building, the Town of Dunn, and Pleasant Springs Town Halls.
